Authorization for Embalming
The undersigned represents to SouthCare Cremation & Funeral Society that he/she is the surviving spouse or the next of kin of the decedent, or is the legal representative of the decedent. As such, he/she has the legal right to the direct disposition of the body of the decedent. The undersigned acknowledges that the authorization encompasses permission to embalm at the Funeral Home facility or at another facility equipped for embalming.
